Smith Island Cdp is a census designated place in the Somerset County, Maryland, United States. In 2000, the population was 364. It is the 36th least populous in the U.S. state of Maryland. It has total area of 9.18 sq mi (23.78 square kilometre), of which, 4.45 square miles (11.53 square kilometre) of it is land and 4.72 square miles (12.22 square kilometre) of it is water, makes it the 68th largest by total area in Maryland. It is ranks fifth in the size of its inland waterways in Maryland.

As of the census of 2000, there were 364 residents, 167 households, and 112 families in the census designated place. The population density was 81.7 people per square mile (31.54/kmē), it is Maryland's seventh least densely populated census designated place. There were 256 housing units at an average density of 57.5 per square mile (22.2/kmē). The average household size was 2.18 persons, and the average family size was 2.69 persons.
