It covers total area of 210.75 square miles (545.84 square kilometre), of which, 130.3 square miles (337.48 square kilometre) of it is land and 80.46 square miles (208.39 square kilometre) of it is water, making it the 44th smallest by size in Virginia. It is ranks ninth in the size of its inland waterways in Virginia.

The population density was 76.2 people per square mile (29.42/kmē), giving it the 76th most densely populated county among the Virginia. In the Middlesex County, 19.2% of the population was under the age of 18, 5.1% were 18 to 24, 22.9% were 25 to 44, 30.3% were 45 to 64, and 22.5% who were 65 or older. For every 100 females there were 92.5 males. For every 100 females age 18 and over, there were 90.5 males.
