Madison County is a county in the United States of North Carolina. According to the 2000 census it had a population of 19635 inhabitants. As of July 2008, the population was estimated at 20432, giving it the 20th smallest population among North Carolina's county. According to the U.S. Census Bureau, the county has a total area of 451.59 mi˛ (1169.61 square kilometre), of which, 449.42 square miles (1163.99 square kilometre) of it is land and 2.17 square miles (5.62 square kilometre) of it is water, makes it the 58th largest by total area in North Carolina.

As of the census of 2000, there were 19635 residents, 8000 households, and 5595 families in the county. The population density was 43.7 people per square mile (16.87/km˛), making it the 14th least densely populated county in North Carolina. There were 9722 housing units at an average density of 21.6 per square mile (8.34/km˛). It has an average household size was 2.34 persons, and the average family size was 2.81 persons. In the county the population was distributed as follows: 21.2% under the age of 18, 10.3% from 18 to 24, 26.5% from 25 to 44, 26% from 45 to 64, and 15.9% who were 65 or older. For every 100 females there were 97.3 males. For every 100 females age 18 and over, there were 93.3 males. The July 2008 population estimate is 20432, The racial or ethnic makeup of the county was 97.23% White, 1.44% Black or African American, 0.29% American Indian, 0.39% Asian, 0.01% Native Hawaiian, and 0.63% from two or more races.
